Public information only · Do not submit protected health information

Get a preliminary AI screening of the records behind your concern. Discuss potential care concerns, what remains unclear and which questions may warrant further clinical or attorney review in a personal 15-minute phone conversation.

Help us focus your review

Tell us what you want to understand.

The more relevant context you provide, the better your reviewer can focus on the questions that matter to you. In your initial request, describe your concern in general terms, explain what you hope to understand and list your most important questions.

Through secure intake, share as much relevant detail as you can: what happened, the sequence of events, what you were told, what remains unclear and which records relate to your concern. Your questions guide the focus; the reviewer’s findings remain independent and based on the records.

Keep patient names, treatment dates, detailed medical information and records out of the public form. We provide secure submission instructions separately. Additional events or a larger record set may require a broader review, which we discuss before work begins.

Before you begin

Your questions, answered.

Will I receive a written report?

No. The $300 service includes preliminary AI screening and a personal 15-minute phone conversation. Written findings, summaries and reports are not included.

Will this tell me whether I have a malpractice case?

This preliminary AI screening can flag questions that warrant further investigation. It does not determine legal merit, promise attorney acceptance or include a formal expert opinion.

What if I have more than 300 pages?

We check whether a focused record set can responsibly address your concern. If a broader review is needed, we explain the appropriate scope and price before you commit. Additional records and new questions require separate authorization.

When does the three-business-day period start?

After we accept the engagement and receive the complete agreed record set. Capacity and your completion date are confirmed before engagement.

Do you obtain records or provide medical or legal advice?

You supply the authorized records. Record retrieval, diagnosis, treatment recommendations, legal advice, physician opinions and testimony are outside this service. Additional professional work is separately scoped and quoted.
